How to Sign Up
Option 1: Invitation from Our Office
You’ll receive an email invitation from FollowMyHealth after your first visit.
Click the link in your invitation to create your account.
(If you have an existing FollowMyHealth account, log in.)Follow the prompts to verify your identity and connect to Integrative Family Medicine of Iowa.
Option 2: Self-Enroll
Visit www.followmyhealth.com or download the FollowMyHealth app.
Click “Sign Up” and search for Integrative Family Medicine of Iowa.
Complete the registration steps and verify your information.

Access for Minors and Young Adults
We take patient privacy seriously and comply with HIPAA and Iowa state regulations.
Parents/Guardians of minors (ages 0–12) may request a proxy account to manage their child’s health information through FollowMyHealth.
Adolescents (ages 13–17) must create their own FollowMyHealth adolescent account. To comply with privacy laws, parents or guardians will not have automatic access to the adolescent’s information. However, the adolescent may choose to grant access to a parent or guardian through the “My Connections” section within their portal.
Adults (18+) must create and manage their own individual accounts.

FollowMyHealth is best for quick, non-urgent communication with your care team. You can use it to:
Ask short follow-up questions after a recent visit
Request medication refills
Provide brief health updates (such as how you’re responding to a treatment)
Access lab results or visit summaries
If you have new symptoms, an in-depth concern, or need a detailed discussion, please schedule an appointment instead of sending a message. This helps us give you the thoughtful, comprehensive care you deserve.
💡 Tip: Your personalized care plan, recommendations, and visit summary are all available in your FollowMyHealth account under “My Health” → “Documents” or “Visit Notes.” If you forget details from your appointment, check there first before sending a message.

Yes. You can request medication refills directly through your FollowMyHealth account under “Medications.”
Please allow 2–3 business days for refill requests to be processed.
Note: Some refills may require an appointment or lab work before renewal — we’ll contact you if that’s needed. -
FollowMyHealth will send an email or mobile notification when new information is available.
Make sure notifications are enabled in your account settings so you never miss an update from your care team. -
Most test results will be available in FollowMyHealth within a few days of completion. Your provider may contact you directly for any results that require discussion or follow-up.
